Breast Cancer Survivorship Survey

Breast Cancer survivorship is an intergral part of the cancer continuum. Survivorship begins at diagnosis, therefor it important to understand where there are opportunities for improvement in the patients cancer journey. The Healing Consciousness Foundation's goal is to identify the opportunities for improvement and find thoses instances and work to improve them. This survey will serve as a starting point for that endeavor.
